Let's talk about referrals. What is it and why do you need it.
Everything you need to know about referrals.

What is a referral?

A referral is a way for a school, service provider, parent, or guardian to connect a young person with our Youth Support Program (YSP).

It helps us understand the young person’s situation so we can provide the right support. Every person is unique and so is their situation.

Referrals make sure we have the right information and consent to begin working with the young person.

How to refer a young person

  • Download the Referral Form and Consent Form

  • Complete both forms with the young person and/or their guardian

  • Email the completed forms to our Youth Support team

Email: ysp.referral@anglicare.net

Why do we need a referral?

Because our programs works closely with young people and other services, we need a referral to:

  • Understand the young person’s needs and circumstances

  • Make sure the young person agrees to receive support

  • Ensure we have permission to share information with relevant services

  • Connect them to the right supports as quickly as possible

This helps us provide safe, coordinated and effective support.

If you need assistance with the referral process, please contact us for FREE at 1800 787 285 or visit our office during operating hours for support.

Important

We cannot process referrals without a completed and signed Client Consent to Referral section from the young person and/or their guardian.

This ensures the young person understands and agrees to the referral.

Transitional Housing Program (THP)

A pathway to transition from care to independent living in partnership with Child Safety Services

THP placements are made through Child Safety, and can be either shared housing or an outreach placement, with practical support for moderate to complex needs.

While in the program, you will be supported to:

  • Build practical life skills and confidence

  • Strengthen relationships and connections with family, culture, and community

  • Stay engaged with school, training, or work

  • Access health, housing, income, and other essential services

Your placement and support plan is outlined by Child Safety and reviewed regularly to ensure you get the right help at the right time.

Our approach is evidence-based, guided by child development, attachment, trauma, and grief and loss. We work closely with Child Safety to help you stay connected to the people and culture that matter to you.
